(B) Employers who will evacuate their employees from the danger area when an emergency occurs, and who do not permit any of their employees to assist in handling the emergency, are exempt from the requirements of this subsection if they provide an emergency action plan complying with 8 CCR 3220 of the General Industry Safety Orders. (2) Elements of an emergency response plan: The employer shall develop an emergency response plan for emergencies which shall address, as a minimum, the following: (A) Pre-emergency planning. (B) Personnel roles, lines of authority, and communication. (C) Emergency recognition and prevention. (D) Safe distances and places of refuge. (E) Site security and control. (F) Evacuation routes and procedures. (G) Decontamination procedures which are not covered by the site safety and health plan. (H) Emergency medical treatment and first aid. (I) Emergency alerting and response procedures. (J) Critique of response and follow-up. (K) Personal protective equipment (PPE) and emergency equipment. (3) Procedures for handling emergency incidents. (A) In addition to the elements for the emergency response plan required in subsection (1)(2), the following elements shall be included for emergency response plans: 1. Site topography, layout, and prevailing weather conditions. 2. Procedures for reporting incidents to local, state, and federal governmental agencies. (B) The emergency response plan shall be a separate section of the Site Safety and Health Plan. (C) The emergency response plan shall be compatible and integrated with the disaster, fire and/or emergency response plans of local, state, and federal agencies. Hazard Assessment for PPE (Safety Person or designated person), in conjunction with Supervisors, will conduct a walk-through survey of each work area to identify sources of work hazards. Each survey will be documented using the Hazard Assessment Certification Form, which identifies the work area surveyed, the person conducting the survey, findings of potential hazards, and date of the survey. (Safety Person or designated person) will keep the forms in the (Specify exact location, e.g., your company's business files). (Safety Person or designated person) will conduct, review, and update the hazard assessment for PPE whenever • a job changes • new equipment or process is installed • there has been an accident - whenever a supervisor or employee requests it • or at least every year Any new PPE requirements that are developed will be added into (Name of your business)'s written accident prevention program. ## B. Selection of PPE Once the hazards of a workplace have been identified, (Safety Person or designated person) will determine if the hazards can first be eliminated or reduced by methods other than PPE, i.e., methods that do not rely on employee behavior, such as engineering controls (refer to Appendix B - Controlling Hazards).
