Test Equipment to assure power source has been removed by depressing the "Start" or On" Switch. 4. Perform required operations. 5. Replace all guards removed. 6. Remove Lock & Plug Box and Tag. 7. Inspect power cord and socket before plugging equipment into power source. Any defects must be repaired before placing the equipment back in service. NOTE: Occasionally used equipment may be unplugged from power source when not in use. ## SOP: LOTO Procedures Involving More Than One Employee In the preceding SOPs, if more than one Employee is assigned to a task requiring a lock and tag out, each must also place his or her own lock and tag on the energy isolating device(s). ## SOP: Management's Removal of Lock and Tag Out Only the Employee that locks and tags out machinery, equipment or processes may remove his/her lock and tag. However, should the Employee leave the facility before removing his/her lock and tag, the Maintenance Manager may remove the lock and tag. The Maintenance Manager must be assured that all tools have been removed, all guards have been replaced and all Employees are free from any hazard before the lock and tag are removed and the machinery, equipment or process are returned to service. Notification of the employee who placed the lock is required prior to lock removal. ## Contractors Contractors, working on company property and equipment must use this Lockout - Tagout procedure while servicing or maintaining equipment, machinery or processes.

Harnesses keep workers safe from fall related injuries, which are the number one cause of jobsite accidents. Keeping your equipment inspected and in working order is the single most important factor for safety. Here are some tips which will help. ## INSPECTING YOUR HARNESS You must inspect all components (including hardware, lanyards, and positioning harnesses or full body harnesses depending on which system is used) of personal fall arrest systems, personal fall restraint systems and positioning device systems prior to each use according to manufacturer's specifications for mildew, wear, damage, and other deterioration. You must remove defective components from service if their function or strength has been adversely affected. (WAC inspection criteria) Webbing: Look for: • Cuts, nicks and tears • Broken fibers or cracks • - Overall deterioration • Fraying and abrasions • Hard or shiny spots ## Stitching: Look for: • Pulled stitches - Missing stitching - Cut stitches • Discoloration of stitches ## Hardware: Look for: • Distortion (twists, bends) • Rough or sharp edges • Rust or corrosion • Broken/distorted grommets • Modifications by users ## CLEANING AND STORAGE • Wipe off all surface dirt with a sponge dampened in plain water. Squeeze the sponge dry. Dip the sponge in a mild solution of water and mild detergent. Work up a thick lather, with a vigorous back and forth motion. Furthermore, there are some tasks, such as machine or equipment inspection, which may either constitute "servicing and/or maintenance" or "normal production operation" activities depending upon the specific circumstances of the work tasks. The purpose or function of the activity determines which standard applies. If the inspection activity is conducted to determine product quality or it is functionally related to the product, then it is a normal production operation. Conversely, if the inspection is performed to troubleshoot a mechanical problem or determine the adequacy of an equipment or machine repair, then the inspection is a “servicing and/or maintenance” activity that is addressed by the LOTO standard.
