Published by SALUS

A behavior-based safety observation form is used to document safe and at-risk behaviors to comply with safety regulations and promote a proactive safety culture. The form should facilitate the observation of employees to verify if they are following procedures and if the workplace conforms to legal and organizational requirements. [8]

Key components to include in a behavior-based safety observation form:

  • Date, time, and location of the observation
  • Name and job title of the observer
  • Description of the task being observed
  • Specific behaviors observed (both safe and at-risk)
  • Classification of the risk level associated with at-risk behaviors
  • Recommended corrective actions
  • Responsible person for implementing corrective actions
  • Due date for corrective actions
  • Action status
  • Space for additional notes and comments

[5] To ensure compliance with safety regulations, the form should:

  • Reference specific regulatory requirements related to the observed behaviors.
  • Include questions about the availability and use of required PPE.
  • Address emergency preparedness and communication systems.
  • Verify that safety rules are visibly posted and provided to employees.

[7] [6] [4] [3] To promote a proactive safety culture, the form should:

  • Focus on identifying and reinforcing safe behaviors, not just identifying at-risk behaviors.
  • Encourage open communication and feedback between observers and employees.
  • Be used as a tool for coaching and mentoring, not for disciplinary action.
  • Ensure that observations are conducted regularly and consistently.
  • Provide a mechanism for tracking and trending observation data to identify areas for improvement.

Additionally, consider the following aspects when developing the form:

  • Housekeeping: Ensure aisles are clear and workstations are not cluttered.
  • Equipment Guarding: Verify that equipment has proper guarding and emergency stop buttons are accessible.
  • Training and Competency: Confirm that employees are trained in proper use methods and that training is documented.
  • PPE: Check for the use of safety glasses, face shields, earplugs/earmuffs, hard hats, gloves, and steel-toed shoes/boots.
  • Material Handling: Assess whether employees are trained on proper lifting and material handling techniques.
  • Chemical Safety: Ensure employees are not exposed to hazardous substances without proper HazCom programs and SDSs.
  • Emergency Preparedness: Confirm the presence of trained CPR/First Aid responders and adequate first aid supplies.

# COR® 2020 Observation Questions IHSA COR® 2020 Observation Questions NATIONAL COR IHSA.ca Work Safe for Life ## Observations The purpose of conducting observations is to verify the information gathered during the documentation review and the interviews. Specifically, the auditor is visually confirming whether the workers are following procedures and/or whether the workplace/site conforms to the legal and organization OHSMS requirements. For example, a health and safety policy is required to be posted. This can be verified through observation and full marks can be granted if observed. Enter your detailed observation notes in the auditor's notes section of the Audit Tool. The worksites included in the audit must be representative of the overall organization operations. To determine the number of worksites to visit refer to the Worksite Observations Guidelines table. In addition to these active sites the main office/shop visit must be included in the scope of the audit. <table><tr><th colspan="2">Worksite Observations Guidelines</th></tr><tr><td>Active Sites</td><td>Minimum Number of Sites to be Visited*</td></tr><tr><td>1</td><td>Site must be visited</td></tr><tr><td>2</td><td>Both sites must be visited</td></tr><tr><td>3-4</td><td>At least 2 sites must be visited</td></tr><tr><td>5-8</td><td>At least 3 sites must be visited</td></tr><tr><td>9-30</td><td>At least one third (1/3) of the sites must be visited</td></tr><tr><td>30+</td><td>Employer/auditor must contact IHSA to determine representative sampling for worksite observation</td></tr></table> * The main office/shop must be included in addition to these worksites.
