# WORKERS Killed While WORKING WITH GRANITE (cont.) FACE the Facts Haz Alert Page 2 ## A JOB HAZARD ANALYSIS SHOULD BE CONDUCTED EACH DAY BEFORE WORK COMMENCES. A hazard assessment of the job site should be performed before work commences in- cluding hazard awareness and appropriate control measures. EMPLOYEES SHOULD BE TRAINED IN PROPER MATERIAL HANDLING PROCEDURES THAT INCLUDE THE HANDLING AND TRANSPORT OF GRANITE SLABS. A standard operating procedure (SOP) needs to be developed for the transport, and re- trieval of granite slabs. The SOP needs to address receiving and retrieval, appropriate slab cart or rack type for typical sizes and weights of granite slabs, slab rack storage, and avoidance of transport hazards such as lack of support pins, individual compart- ments, etc. When available, use material han- dling equipment, such as gantry cranes or fork lifts, with the PROPER attachments, to lift and move slabs. Never stand under or next to slabs that are being moved. Never manually support large stone slabs. USE SLAB CARTS OR SLAB RACKS TO TRANSPORT GRANITE SLABS AND USE TIE-DOWNS TO SE- CURE THE GRANITE SLABS TO THE FORKLIFT. Slab carts should be designed by registered pro- fessional engineers to account for anticipated load capacities, slab sizes, slab quantities to be transported, and for transfer by forklift. Slab racks could be designed with fixed support pins and individual compartments for each slab. Em- ployers should also ensure that the slab racks are used according to manufacturers' specifications. ## WORK SHOULD ONLY BE PERFORMED WHEN THE GENERAL CONTRACTOR HAS A COMPETENT PERSON ON THE JOB SITE. The general contractor was not on the job site on the day of one of the incidents and it is unknown if a competent person was on the site of the other incident. # California Department of Health Services FACE Report 94CA01401 Elevator Maintenance Worker Dies from Fall in an Elevator Shaft in California SUMMARY A 34-year-old white, non-Hispanic, male elevator maintenance worker (the victim) died after falling approximately 30 feet into an elevator shaft. At the time of the incident, the decedent and two coworkers were pulling a hydraulic piston out from the bottom of the elevator shaft so that a new liner could be installed. Prior to performing this operation, the workers had installed an electrically powered, base mounted capstan (a revolving barrel on a vertical axis for winding cable) or cathead in the bottom of the elevator shaft which was to be used as a hoist to lift the piston up to the top of the shaft. Co-worker #2 had been sent to the fourth floor so that he could inform the other workers when the piston reached the top of the shaft. The victim was working from the first floor and co-worker #1 was at the bottom of the shaft. Co-worker #2 yelled when the piston hit the top of the elevator shaft but his co-workers apparently did not hear him. Co- worker #1 continued in his efforts to raise the piston which resulted in the capstan being pulled out from the floor of the shaft where it had been anchored. It flew up into the shaft and the piston fell back down to the bottom of shaft. Co-worker #1 became entangled in the hoisting ropes and was pulled up into the air. The victim, stationed on the first floor, apparently looked into the shaft to help and was hit in the head by the capstan. The victim then fell to the bottom of the shaft. Both co-workers pulled the victim out from the elevator shaft and began First Aid. The security guard called 911 and fire department paramedics were summoned to the scene. An on- site examination revealed multiple fractures of the skull and jaw. The decedent was pronounced dead at the scene by fire department paramedics. He was employed by an excavation contractor and had 15 years of experience driving dump trucks with trailers. He drove the truck to his employer's property to drop off the attached trailer. He backed up into a parking spot at the top of a slight incline, set the trailer parking brake, and exited the truck. When he unhooked the trailer, the truck started to roll downhill. He ran after it and got onto the driver's side running board, attempting to get in the cab to stop the truck. REPORT DATE: December 9, 2024 INCIDENT DATE: June 6, 2024 WORKER: 76 years old ## INDUSTRY: Site Preparation Contractors ## OCCUPATION: Dump Truck Driver ## SCENE: Trailer parking area ## EVENT TYPE: Caught between vehicle Dump truck at fatality scene. He was then dragged until the truck finally stopped 150 feet from the parking area. The employer's son was working nearby and saw he was pinned against the hillside. He called his father and they used an excavator to pull the truck off the driver. They called 911 but found he was already deceased when they tried to render first aid. Temporary Laser-Controlled Area Should overriding interlocks become necessary during periods of special training, service, or maintenance, and access to Class IIIB or Class IV lasers is required, a temporary laser-controlled area shall be devised following specific procedures approved by the LSO.
