FACE/WA Fatality Assessment FATALITY NARRATIVE and Control Evaluation Skid-Steer Loader Operator Crushed Between Backhoe Attachment and Cab* Industry: Electrical contractors Release Date: February 3, 2012 Task: Operating skid steer loader up ramp to trailer Incident Date: March 1, 2010 Occupation: Journeyman equipment operator Case No.: 10WA01801 Type of Incident: Crushed between SHARP Report No.: 71-106-2011 On March 1, 2010, a journeyman equipment operator was crushed between the backhoe attachment and the cab of a skid-steer loader. The 47-year-old operator was hired from the local IUOE hall by an electrical contractor to perform work at a power company's substation. He had worked for this employer on and off for several years; this was his first day at this jobsite. The victim and an employee of the contractor were Incident scene loading equipment and tools to move to another job site. The victim's task was to load a skid-steer loader with a backhoe attachment on to an equipment trailer for transport. As the victim moved the loader toward the trailer ramps, the backhoe attachment dragged on the ramps, preventing it from being loaded. He got down from the machine and connected the backhoe's hydraulic hoses and then got back into the cab. He may have intended to use the bucket of the backhoe attachment to lift the loader's front end to prevent the attachment from dragging while moving up the ramp. The victim was seated in the cab with the safety restraint bar in the down position (it must be down to operate the machine). He then stood, perhaps for a better view, and reached for the backhoe attachment operator's seat in front of him. At the same time his hand hit a button which engaged the bucket and his left foot became stuck on the control that activated the backhoe attachment. The bucket then moved toward him, pinning him between the exterior seat and the cab frame. The victim was taken to a hospital where he died of blunt force trauma to th

Workers who operate or work near this equipment are at risk for injuries from falling off or being struck by the equipment. ## Ron's Story Ron and two coworkers were removing a fence (with stakes) that surrounded a housing development drainage pond. A loader was being used to pull up the stakes, because overgrowth made it difficult to remove them by hand. As the operator was raising the lift arms to pull the third stake, the loader tipped forward. To stabilize the machine, the operator lowered the bucket. Just then, Ron (who had been standing in front and to the side of the loader) slipped and fell beneath the bucket. # Daily Safety Check ## CHECK YOUR TRUCK BEFORE STARTING WORK You must make sure your truck is safe to use. ## 1. Walk around your truck and check it over • Check to make sure the battery is charged, water level is OK and vent caps are in place, unless a sealed, maintenance-free battery has been selected. Don't use an open flame to check the battery. • See that all wheels are in good condition. • Check that both forks are secure and not bent, cracked or badly worn. • Inspect lift chains for damage. • See that the load backrest is in place and secure, if your truck has that option. • Look under the truck for signs of hydraulic leaks. • Test the horn. • Make sure the power disconnect works. • Check that all controls work smoothly. ## 2. Test drive your truck in an uncongested area. • Test the hydraulic functions. • Check that steering is smooth and easy. • Operate the truck slowly in both directions. • Operate through the full speed range in both forward and reverse. • Check braking distance in forward and reverse at a slow speed. Load size and floor conditions can affect these distances. • Know the distance it takes to stop before you start working. If the braking distance is too long to stop safely, don't drive the truck. ## If anything doesn't look, work or feel right, don't use the truck. Report the problem to your supervisor. WASHINGTON State FACE Program Fatality Assessment & Control Evaluation CONSTRUCTION # FATALITY NARRATIVE FACE National Institute and Health NIOSH INCIDENT FACTS # Skid Steer Loader Operator Crushed REPORT #: 71-166-2018 ## SUMMARY A 54-year-old skid steer loader operator died when he was pinned between the lift arms cross bar and the frame. The victim had 25 years' experience operating many types of equipment, including skid steer loaders. His employer was a site preparation subcontractor responsible for digging trenches for water pipes. He had been working at the job site for two weeks. On the day of the incident, the site foreman sent the victim and another worker to get a skid steer loader to backfill trenches. The other worker gave the victim the key to the loader and told him the "dead-man" (safety interlock) switch for the lap bar was not working properly. This switch prevents the machine from starting unless the bar is in the "down" position. As the other worker was walking away, he heard the machine start. He turned around and saw the victim pinned between the lift arms cross bar and the loader's frame. The worker ran to the loader and tried to move the arms off the victim, but the controls did not respond. He called his supervisor and then 911. Several workers rushed to the scene and after multiple tries were able to extract the unconscious victim. He did not regain consciousness and died four days later. Investigators were unable to determine why the lift arms lowered on the victim. The lap bar safety interlock was found to be not working. The victim was able to start the machine with the lap bar in the "up" position, instead of the normal "down" position. He started the loader while standing on the ground outside of it. When he reached in to start the machine he may have unintentionally activated the boom arm's control, causing the arms to lower.
